Please use this form below to submit a referral for the TAG program.

Note that a referral initiates a review process but does not guarantee placement into the TAG program. The TAG team will evaluate all available evidence to determine whether further testing or placement is appropriate for the student at this time.

Sheridan School District staff should use the Intent to File form to notify administration of their intent to file for a specific grant before beginning to work on the grant and after speaking with their building principal. 

Please complete the Fundraising Activity Request form no less than 30 days prior to initiating the fund-raising activity.
